In-Home Functional OT Assessment

Using the Canadian Model of Occupational Performance and Engagement as a guide, our OTs conduct a full assessment to evaluate the relationship between you, your occupations, and your home environment.

You will be assessed using standardized assessments and observations that evaluate your physical, cognitive, and psychosocial status. A comprehensive report will be written to analyze the relationship between the person, their occupations, and environment to ensure your safety.

Ergonomic Assessment

Ergonomic assessments are completed to ensure your workspace is properly set up to prevent injuries and facilitate a return to work following an injury. These assessments can also be completed to prevent an injury from worsening.

We use the Canadian Model of Occupational Performance and Engagement as the framework to evaluate the fit between you, your occupations, and your work environment.

Cognitive Functioning Assessment

A series of standardized assessments will be used to measure attention, memory, executive functioning, and perceptual skills. Additionally, an interview and observations will be made regarding your functional ability.

Depending on the results, a treatment approach will be tailored to help remediate or compensate for cognitive deficits.

Assistive Devices Program (ADP)

Our OT is an ADP authorizer, helping you access government funding for mobility aids, bathroom equipment, and other assistive devices to support your independence.
